Claim Analysis Group provides independent claim quality analysis designed to assess whether a file has been investigated thoroughly, documented clearly, and supported in a way that can withstand internal scrutiny, regulatory review, or legal challenge.
This is not a re-investigation.
It is a structured, objective evaluation of the existing claim file, examining whether the evidence, investigative work, documentation, and reasoning adequately support the position being considered.
Our analysis identifies strengths, evidentiary or documentation gaps, unresolved inconsistencies, and areas that may require clarification or reinforcement before significant claim decisions are finalized or defended

What We Evaluate
Evidentiary Support: Whether material findings and conclusions are adequately supported by the information contained within the file.
Investigative Completeness: Whether relevant issues appear to have been sufficiently explored and whether material questions remain unresolved.
Documentation Quality: Whether the file clearly distinguishes reported information, verified facts, analytical observations, and conclusions.
Consistency & Reasoning: Whether the documented reasoning is logically consistent with the available evidence and investigative findings.
Unresolved Gaps: Identification of missing support, conflicting information, or areas that may warrant clarification or additional investigation.
Overall File Defensibility: An independent assessment of the strengths and weaknesses affecting the file’s ability to clearly support the carrier’s position.

Claim Defensibility Analysis may be appropriate when:
A claim presents elevated complexity or uncertainty
A significant claim decision is approaching
A file is approaching escalation, dispute, or litigation
Claims or SIU leadership wants an independent review before finalizing a position
An organization is conducting claim quality or SIU effectiveness reviews
Each analysis is delivered in a clear, structured format identifying:
Key observations and findings
Strengths within the existing claim file
Evidentiary, investigative, or documentation gaps
Unresolved inconsistencies or questions
Areas requiring clarification or reinforcement
An independent assessment of overall file defensibility
We strengthen the reasoning inside the file

Claim Analysis Group (CAG) operates independently and outside client systems. Our analysis is based on the claim materials provided for review, allowing us to evaluate the file from an objective perspective without disrupting existing claims or SIU workflows.
Regular review asks: did someone else look at this file?
CAG analysis asks: Does the file actually support the position being taken?
CAG does not adjust claims, determine coverage, or recommend claim acceptance or denial.
Final claim decisions remain fully with the carrier.
